Goddess Fest Minor Participation Waiver & Release of Liability

Event: Goddess Fest 2025

Date & Location: August 23, 2025 at Serenity Forest Elgin

As the parent/legal guardian of the minor named below, I acknowledge and agree to the following:

Voluntary Participation
I understand that Goddess Fest is an event designed primarily for adults and may include workshops, performances, and activities that involve physical movement, sound, art, and wellness practices. I voluntarily allow my child(ren) to attend under my supervision.

Parental Responsibility
I agree that I am solely responsible for the supervision, care, and safety of my minor child(ren) at all times during the event. The organizers, hosts, facilitators, and venue staff are not responsible for providing childcare or supervision.

Assumption of Risk
I understand that participation in any activities may involve risks, including but not limited to, physical movement, emotional response, or interaction with materials (such as art supplies or wellness tools). I voluntarily assume all risks, known and unknown, on behalf of my minor child(ren).

Release of Liability
I release, waive, and hold harmless the organizers of Goddess Fest, its affiliates, facilitators, vendors, volunteers, and venue representatives from any and all liability, claims, demands, or causes of action arising out of or related to the participation of my child(ren) in the event.

Medical Consent
In case of emergency, I authorize event staff to seek necessary medical treatment for my child(ren). I understand that I am responsible for any medical expenses incurred.

Acknowledgment of Media Release 
I acknowledge that photos or videos may be taken at Goddess Fest. I consent to my child(ren)’s image being used in event-related media.
